NHS Contracts Breach:Impact on Pharmacies and Patient Access - 0 views

    There is clear evidence that the majority of distance selling pharmacies are operating in breach of their NHS contracts, and a failure to properly regulate 'pseudo' distance selling pharmacies is leading to local pharmacy closures, the Company Chemists' Association has reported. According to the CCA's recent findings, 72 per cent of DSPs dispense over 50 per cent of their prescriptions to patients within a single postcode area within 10 miles of the pharmacy. "Operating within constrained geographical regions, pseudo-DSPs achieve reduced overheads and operational costs by focusing on localised marketing and medicine delivery. They compete against local brick-and-mortar pharmacies, causing them to lose vital trade," the CCA claimed, based on its recent survey - The Impact of Pseudo Distance Selling Pharmacies. "As per their terms of service, DSPs are obligated to provide prescription delivery nationwide, extending beyond local patients," said the CCA, adding that the financial savings resulting from the lack of patient access are balanced by the necessity to function on a national scale.

Pharmacists raised questions after the Pharmaceutical Services Negotiating Committee (PSNC)'s 2023 Pharmacy Pressures Survey confirmed the ongoing pressures and health issues faced by the pharmacies. Pharmacists are not all shocked by the PSNC's survey report as they feel the same as what has been reported related to their businesses and health. They hope the government listens and work with them to find resolutions. "We are bullied into a corner," said Salim Jetha Chairman, Avicenna. "Unlike other industries, we can't increase our prices. Most of the daily calls I get from Independents is about financial health of their business and any cost cutting would be detrimental to patient care. Urgent holistic review is required." Bristol pharmacist Ade Williams said: "The report is a dire indictment, and I would also warn, likely an underestimate of the extent and detrimental impact of the ongoings pressures and squeeze on Community pharmacies." "If the closest interface of the NHS to communities and patients is so distressed, what does that mean for those that need and depend on us? We are notoriously very stoic, so this is a warning light, which, taken with workforce pressures, market-exit activity, and other reports raising concern about wellbeing and stress, must beg the question; what will it take to get help before the sector and individuals break?" he questioned. The survey results don't surprise Kent-based community pharmacist Amish Patel. He said, "I have been feeling exactly what has been reported for far too long. I'm burnt out and would say beginning to suffer with my own health because of it. Now it's for PSNC to talk to government, and government to listen and work with us to find resolutions."

NPA survey:Independent pharmacists witness negative cashflow - 0 views

    The National Pharmacy Association (NPA) survey revealed that 93 per cent of respondents experienced 'at least one month' of negative cash flow across their pharmacy business in 2022. Nine out of 10 pharmacy owners responding to NPA survey said they made a net loss dispensing medicines for the NHS during 2022, for at least one month of the year. According to the poll, 48 per cent of them lost money on this core NHS service for six months or more. 45 per cent said their overall outgoings had exceeded overall income in at least six months of the year. NPA Chair Andrew Lane said: "This survey shows the bleak financial reality facing many independent pharmacies after years of underfunding. Dispensing at a loss and negative cashflow is clearly unsustainable." "This funding crisis must be addressed urgently or pharmacies will fall into a spiral of declining services and ultimately widespread closures, he added. "Tragically, the story is playing out very much in line with independent research commissioned by the NPA last year, which warned of a nationwide financial emergency in our sector."

    The large majority of adults in the England knew that flu jabs are available in many pharmacies, just over half were aware of the GP Community Pharmacist Consultation Service or the New Medicine Service, according to a new survey by the National Pharmacy Association (NPA). The new survey revealed the need to raise the public awareness of key NHS services such as the New Medicine Service. According to the survey, 56 per cent of those asked believed that pharmacies in England offer NHS consultations for people newly prescribed a medicine for a long-term condition. While 48 per cent believed that many pharmacies in England offer blood pressure monitoring (NHS or private). 51 per cent believed it to be true that GPs can formally refer patients for same-day clinical advice from their local pharmacist about minor ailments.

Community Pharmacies Handle 69 Million Unfunded Consultations Annually - Urgent Call fo... - 0 views

    Pharmacies are valuable healthcare assets, meeting increasing public demand. However, a substantial portion of their capacity is consumed by unfunded work. According to Community Pharmacy England's (CPE) 2024 Pharmacy Advice Audit, community pharmacies conduct over 1.3 million unfunded consultations each week, equating to 69 million annually. The average pharmacy completes around 22 of these consultations daily, each taking approximately 5.6 minutes of staff time -adding up to over two hours daily. This represents nearly 50 per cent more consultations than in 2020 and a 78 per cent increase in time spent on unfunded healthcare advice since the 2020 audit. The audit, which surveyed over a third (3,916) of community pharmacies in England earlier this year, also highlights that pharmacies handle nearly 150,000 informal referrals from GPs and NHS 111 each week. All these referrals could have been routed through the Pharmacy First service, which would have potentially secured over £115 million in funding for the sector, the CPE noted.

Pharmacy teams were added with extra burden to manage and resolve these issues. Despite these challenges, 85 per cent of patients expressed satisfaction with pharmacy services, reflecting the dedication and value of pharmacists and pharmacy teams in the community.

    Community Pharmacy England (CPE) has explained MPs on instability that puts operational pressures on pharmacies, financial pressures on businesses at a Parliamentary drop-in event held on Monday (10 July). The association has been in Parliament today alongside patient representatives and others to highlight our ongoing concerns about medicines supply to MPs. It said: "Medicines supply remains a critical issue for community pharmacies with disruption causing problems both accessing medicines and procuring them cost effectively." In CPE's recent sector polling, community pharmacy owners rated medicines supply instability as being the most severe pressure facing their businesses. This echoed the results of CPE's Pressures Survey which found 97% of pharmacy owners survey are facing significant increases in wholesaler and medicine supply issues, with 71% saying this was leading to delays in prescriptions being issued. During the Parliamentary drop-in event, CPE talked MPs through the issues and set out what it believe needs to happen to resolve them, calling for- Reform of Serious Shortage Protocols; Allowing generic substitution; An overhaul of the concessions system; and a strategic Government review of medicine supply and pricing with a shift to focusing on how to improve the functioning of the supply chain rather than solely on the drive to depress prices and margins.

    Nine in ten people surveyed by Ipsos on behalf on NHS positively rated the advice they received from their local pharmacies. The results from Ipsos found that the vast majority of patients (91%) who had used a community pharmacy in the previous year for advice about medicines, a health problem or injury, or what health service they should use said they received good advice. The research comes as more than 11,000 pharmacies in England can now access training to spot signs of cancer as part of a new drive to catch tumours earlier when they are easier to treat. Thorrun Govind, Chair of the Royal Pharmaceutical Society English Pharmacy Board, said: "Community pharmacists are working exceptionally hard to make sure that the public gets the right advice at the right time. This new data shows the public appreciate both the accessibility and quality of advice they receive from community pharmacists. "I am delighted that the Royal Pharmaceutical Society is working with the NHS to deliver professional development for community pharmacists to further enhance their clinical consultation skills.

"With more than 38 million patients in the UK waiting more than two weeks for a GP appointment, it is unsurprising that 34 per cent of people surveyed asked a pharmacist for advice over their primary care physician/doctor in the last 12 months," 2san said. In Wales, over a third of people prefer their community pharmacist for support and advice, while those aged 65 and above still tend to consult their primary care physician.

In June, ACS commissioned Yonder Consulting to conduct a poll surveying the opinions of 1,000 people across the UK. The survey assessed how these individuals value 16 different services in their local area, defined as the region within a 15-minute walk from their homes.     The Royal Pharmaceutical Society (RPS) and Pharmacist Support hosted a roundtable on the impact of pharmacy workforce wellbeing on patient safety on Wednesday (17 May). It also released its annual Workforce Wellbeing Survey which showed continued pressures on pharmacy teams. The discussion explored the actions needed to support staff so they can continue providing safe and effective patient care and included representatives from the NHS, professional bodies, employers, trade unions, education and regulators. A report of the roundtable will be published in the summer. Amandeep Doll, RPS Head of Professional Belonging, said: "We know that pharmacy teams go above and beyond for their patients, but are also under enormous pressure. "Now more than ever, we need to strive to make the pharmacy profession more inclusive and ensure that everyone's wellbeing is supported. "It is vital that we do all we can to encourage people into pharmacy and to support them so they can enhance their skills, develop their careers, and continue to deliver high-quality patient care. "This discussion was a welcome step and showed that making a difference for staff wellbeing requires a concerted effort from stakeholders across the whole of pharmacy.

    Community pharmacies are located within the very hotspots of peoples' communities, yet a staggering 720 individual businesses have stopped trading since 2015. It has been called a "national disgrace and a betrayal to the elderly and the vulnerable" by sector leaders. The funding crisis has been compounded by a gruelling 18-24 months post-Covid, both have which have placed immense pressure on the retail pharmacy sector. The population is aging and with this comes patients living with a range of long-term health conditions. Medication regimes are becoming ever more complex and as a result, maintaining prescription and medication adherence services is challenging to say the least. Yet the public feel great warmth towards their local pharmacy businesses, and this positive attitude only increased during the pandemic. In fact, in a recent report launched by Omnicell on the Changing Role of the Pharmacist, a survey of public opinion found that a third (32 per cent) said they perception of their local pharmacy had changed for the better since the start of the pandemic and nearly half (44 per cent) said they are more familiar with their local pharmacy services since the start of COVID. This has meant that the role of the pharmacist has also changed and evolved; in part due to a national need and drive for more patients to be seen and supported clinically by pharmacists. This change of service provider role has slowly been developing over the last couple of years. Pharmacists are currently carrying out 65 million consultations a year - a staggering rise of 44 per cent in the last 24 months alone.

The poll found that approximately one-fifth of British consumers (18 per cent) have used a pharmacy delivery service in the past year. However, more than double that figure (40 per cent) expressed their intent to use such a service in the next 12 months if it were available, suggesting an opportunity for growth in this sector. This difference is especially pronounced in the Midlands, Scotland, and Wales, where the potential demand more than triples actual use. Compared to other regions, the current use of pharmacy delivery services is notably low in the Midlands, standing at 12 per cent, and in Wales and Scotland, where the collective usage rate stands at 14 per cent.

GPhC Seeks Pharmacy Professionals' Feedback on Revalidation Process | UK 2024 - 0 views

    The General Pharmaceutical Council (GPhC) is inviting pharmacy technicians and pharmacists who have undergone revalidation since October 2022 to share their experiences by participating in a survey. Additionally, pharmacy professionals are encouraged to provide feedback on the impact of revalidation and offer comments and suggestions for improving the process. The Council said: "The purpose of this survey is to ensure that the current revalidation process is effective and meets the needs of professionals and the public." To support this review, the GPhC has appointed the consultancy firm Grant Thornton, which will also help in identifying any gaps and determining areas that may require further development. "The Grant Thornton review will contribute to the development of potential changes to the revalidation framework," it said.

    Too many unreasonable demands placed on pharmacists by employers affect their health and wellbeing at work, a stress and wellbeing survey conducted by The Pharmacists' Defence Association (PDA) Union revealed. The survey, conducted during Autumn 2021, covered views of over 2,000 pharmacists employed by big multiples - Lloyds, Boots, and Well. This survey used questions developed by the Health and Safety Executive (HSE), which enabled the union to understand more about stress and wellbeing in three of the largest community pharmacy multiples. Analysing the survey, the PDA said that some pharmacists working for each of the three employers highlighted "unachievable targets, unrealistic time pressures, and most worrying of all reported numerous instances of bullying and harassment." An overwhelming majority of the respondents reflected personal sacrifices made by the pharmacists to ensure patient services are not impacted by the poor conditions and environments in some pharmacies.

    The Company Chemist Association (CCA) highlighted its concerns around the recruitment of pharmacists into Primary Care Networks as well as the need for additional investment. Commenting on the Ipsos survey findings, the Malcolm Harrison, the Chief executive of the CCA said: "We welcome the findings of the Ipsos survey, confirming that patients value the quality of service and advice community pharmacies routinely provide. "We are very concerned however that this cancer detection pilot, and all other pharmacy services, are at risk if the NHS is not prepared to inject urgently needed funding into the sector." He stated that without substantial investment, "we will see the continued erosion of the service pharmacies can provide." In addition to the need for critical additional funding the association is also calling on the NHS to pause the recruitment of pharmacists into GP surgeries. He said: "Patients are suffering because the demand for pharmacists in England is now significantly greater than that which the existing workforce can deliver."

    More than half of people (52%) do not feel confident in treating backache and less than half (44%) uses community pharmacists' advice as their first step on self-treatable conditions, a self-care attitudes 2022 survey has revealed. PAGB, the consumer healthcare association, surveyed over 2,000 adults across the UK about their attitudes towards self-care and access to health services. According to the survey, the public is in favour of self-care but many people lack the confidence and knowledge to care for the most common self-treatable conditions including backaches, headaches, diarrhoea and constipation. Four in five respondents (79%) said that people should take more responsibility for their own health and three-quarters (73%) believe that the NHS should make self-treatment easier. Behaviours around the current use of pharmacy varied - with less than half (44%) using community pharmacists as their first step in in getting advice on self-treatable conditions - but longer term attitudes were encouraging, with 69% supporting the idea of GPs being able to make referrals to pharmacists. Respondents also supported expanding the role of pharmacists so that their skills can be better utilised.

    The Royal Pharmaceutical Society (RPS) has called its member to to express their views and fill out the Department of Health and Social Care's hub and spoke survey on the proposed model of dispensing. The Society has urged its members to fill out the survey before May 20. RPS president Claire Anderson said: "It is vital that RPS members have their say on issue that impact you in your roles. Currently only single legal entities can make use of this model of dispensing. "Our survey, which opened today, seeks your views and comments and will help to inform the RPS submission to this consultation." The consultation on the proposals to enable all community pharmacies to access hub and spoke dispensing published by the Department of Health and Social Care (DHSC) will run for three months and will close on June 8, 2022.

    The University of the West of England (UWE) is conducting a survey to explore current practice in community pharmacy regarding the management of individuals who present with bladder and/or bowel incontinence. Approximately one in five people experience bladder or bowel leakage and they often do not know those simple conservative strategies can make a difference. A team from the UWE is conducting a study to understand the views on the Pharmacy Role In proMotion of continencE (PRIME); learn from current experiences; and explore what is feasible to develop, for improved bladder and bowel continence promotion. The survey should take around 10-15 minutes to complete and should be completed by one member of staff working within the community pharmacy.
