The National Health Service (NHS) retention programme has benefitted 23 NHS Trusts since it launched in April 2022, with thousands fewer staff leaving frontline roles. NHS data showed that 14,000 fewer staff left the service in the 12 months up to August 2023 (108,890) compared to 122,970 the year before. In a statement released on Thursday, the NHS England said it is expanding the pilot programme across the country to benefit 42 more NHS trusts. The programme is part of the NHS Long Term Workforce Plan ambition to retain up to 128,000 more staff over the next 15 years in addition to training record numbers of doctors, nurses and other healthcare professionals. Under this new initiative, NHS staff are being offered extra flexibility with working hours, clinical 'support squads' have been introduced to help menopausal women at work, and HR 'stay advocates' are designated to identify ways to keep staff on the brink of leaving.

The government has earmarked £200 million to enhance NHS resilience and expedite patient care during the upcoming winter season. This extra amount will bolster the health service during its busiest period, while protecting elective care so we can keep cutting waiting lists, Prime Minister Rishi Sunak has said. "This £200 million investment, assured by the Department of Health and Social Care as new and additional funding, should aid NHS leaders in their preparations and mitigation for what will be a seriously difficult winter period," said Matthew Taylor, Chief Executive of NHS Confederation. "The priority now is swift allocation of funding to local systems for optimal utilisation."

Health leaders have said that delivering faster and more productive patient care would be difficult unless NHS capital budgets are nearly doubled to at least £14.1 billion from the current spending of £7.7 billion. The NHS Confederation has highlighted the impact that low levels of capital investment have had over the last decade in its report published today (29 November). The report, 'Investing to save: The capital requirement for a more sustainable NHS in England', revealed that the UK has lagged behind other comparable countries in terms of health capital investment for more than five decades, resulting in less productivity. It revealed that the NHS now has the sixth lowest number of CT and MRI scanners per million people of the OECD countries.

The National Pharmacy Association (NPA) has welcomed the Chancellor's commitment to increase the NHS budget, hoping that 'community pharmacies benefit from this investment'. The association is urging the government to address years of underfunding so that community pharmacies can avoid staff lay-offs to cover the increasing costs of the national living wage. The government's uplift of the National Living Wage was confirmed in its latest budget announcement to tackle the cost of living crisis. The NHS budget will also be increased in each of the next two years by £3.3bn. A recent NPA commissioned report by Professor David Taylor from University College London predicted that wage inflation and other cost pressures could combine with funding cuts to lead to cut-backs and pharmacy closures.

Rise in deep poverty, cost-of-living crisis, and high pressure on NHS services have worsened the situation, according to a study published by The King's Fund this week. The report underscored that poverty and deprivation contribute to a greater prevalence of diseases, difficulties in accessing health care, late or delayed treatment, and worse health outcomes. These challenges could be seen across various NHS services, spanning from emergency care to dental services Additionally, it revealed that 30 per cent of people living in the most deprived areas have turned to 999, 111, A&E or a walk-in centre because they could not access a GP appointment. In 2016, the Joseph Rowntree Foundation (JRF) estimated the cost of poverty on health care at £29 billion (£34 billion in current prices). As the proportion of people living in deep poverty has risen, the situation has worsened. In 2021/22, six million people were living in very deep poverty, up from 4.5 million two decades ago. Currently, more than one in five people in the UK are estimated to be living in poverty, the report noted. Deprivation is linked to a range of diet-related health problems, including cardiovascular disease and diabetes, as well as mental illness. According to the report, the depression rate is two times higher among people living in the most deprived areas, compared to the least deprived areas.

Long waiting times for National Health Service (NHS) care are leaving people too sick to work in the UK, which is affecting the country's economy. One in three people missed work in 2022 due to delays in accessing NHS care, revealed a new survey by Pollsters Savanta. The market research consultancy surveyed 2,235 people in the UK this month on behalf of the Liberal Democrats. According to the survey results, 19 per cent of the participants could not go to work as they were waiting for a GP appointment. While 15 per cent of the respondents had to take a long period off work while they waited for treatment or surgery, 12 per cent missed work as they waited for emergency dental treatments. Overall, 36 per cent of those surveyed had missed at least some work because of difficulty getting NHS care.

The NHS Confederation has urged the government to reconsider its 'living with Covid' plan and introduce mitigating actions that will help avoid further critical incidents being declared at the NHS front-line. The organisation said the high rates of Covid is having a major impact on the delivery of health services and slowing down efforts to reduce large waiting lists, noting that the country has been in the grip of another spike in Covid cases resulting from the Omicron BA.2 variant. With more than 20,000 patients now in hospital with Covid (or who have Covid but are in hospital for other reasons) and high staff absences, NHS is facing huge operational challenges, harming efforts to reduce waiting times in other areas, it said.

NHS England's decision to forgo commissioning puberty suppressing hormones (PSH) for children and young people, citing insufficient evidence of their safety and clinical effectiveness, follows a comprehensive review conducted by the National Institute for Health and Care Excellence (NICE) in 2020. The clinical report underscores that "PSH treatment options will not be part of routine commissioning" for people under the age of 18 in England grappling with gender incongruence or dysphoria. The NHS England review analysed nine observational studies on Gonadotrophin Releasing Hormone Analogues (GnRHa) and found no statistically significant difference in gender dysphoria, mental health, body image, and psychosocial functioning among children and adolescents treated with GnRHa. Duncan Rudkin, chief executive of the General Pharmaceutical Council (GPhC), acknowledged the challenges faced by pharmacy professionals regarding prescriptions for puberty suppressing hormones, stating:

Challenges in accessing the NHS waiting list are pushing more and more people toward private-funded alternatives for access, diagnosis and treatment, thereby increasing the demand for private health cover. According to a report published by health data provider LaingBuisson, the demand for private health cover, including medical and dental insurance and cash plans, has surged to its highest levels since 2008. The UK's overall health cover market reached £6.7 billion in 2022, an increase of £385 million from the previous year. During the period from 2020 to 2022, the market experienced annual growth rate of 6.1 per cent, a significant increase compared to the average annual growth rate of 1.7 per cent observed between 2008 and 2019, the research found. A total of 4.2 million people were subscribed to medical cover schemes in 2022. When dependents covered by these policies were included, the number reached 7.3 million - the highest count since 2008. LaingBuisson's analysis showed that medical cover volumes were steady through most of the 2010s, but began to increase alongside rises in the NHS waiting list from 2018.

She told Pharmacy Business reporter that she had to work out something "hand in glove to help them get those vaccinated schemes up and running, as pharmacies were desperate to take part in the vaccination scheme" in her constituency. Describing their unique selling point (USP), Cooper emphasised the convenience and immediacy of pharmacies located in high street areas and warned that pharmacy closures "should be a real wake up call for the government."

The recent industrial action by junior doctors and hospital dental trainees, which is considered the longest strike in NHS history, caused a significant impact on the NHS services. Data published by NHS England showed that 113,779 inpatient and outpatient appointments were rescheduled due to the strike, which began on Wednesday 3 January at 7.00am and finished at 7.00am on Tuesday 9 January. In a September 4 blog post, CPE Chief Executive Janet Morrison emphasised that the relentless pressures show no signs of easing. "Pharmacy businesses, like numerous other sectors, are grappling with the challenges of staying afloat in the current economic climate, compounded by the detrimental effects of historical funding cuts." "The doctors' strike, despite ongoing assurances from the Health Secretary that their pay deal is final, will inevitably exacerbate the demand for both primary and secondary care, leading people to turn to their local pharmacies for assistance," she added. "This, in turn, will heighten the pressure on community pharmacies." "Whatever the coming winter brings I am sure of two things: that community pharmacies are going to be more critical to the health of the population than ever before, and that your representatives here will be fighting on all fronts to get you the help that you need."

As the countdown to the next UK election begins, the King's Fund has identified three priorities to improve public health. The national action would be taken by the future government to fix the "NHS and social care" in the country. The health policy think tank said it would prioritise "improving access to out-of-hospital care", making "careers in health and social care" more attractive and tackling the biggest risk factors affecting people's health. A new Attention-Deficit-Hyperactive disorder (ADHD) taskforce has been launched by National Health Services England (NHSE) to counter the challenges faced by many suffering from the neurodevelopmental condition. The ADHD taskforce aligns with the NHS Long Term Plan's vision to revolutionise mental health services, with a specific emphasis on bolstering community-based care and reducing reliance on inpatient services. The NHS has partnered with the government to tackle the escalating demands including increasing funding, improving system efficiency, altering the service provision model, and clinical prioritisation by bringing together expertise from across a broad range of sectors, including the NHS, education and justice, to help provide a joined-up approach in response to concerns around rising demand. Over the past two decades, there has been a significant expansion in the acknowledgement and successful medical interventions for ADHD, leading to a steady rise in clinical requests throughout the UK.

A recent UNISON survey has unearthed disturbing findings regarding the prevalence of sexual harassment among NHS staff across the UK. With data collected from over 12,000 healthcare workers, the survey sheds light on the challenges faced by frontline workers in their workplace environments. Approximately 10 per cent of healthcare professionals reported instances of sexual harassment during their tenure, with nearly 29 per cent of those experiencing harassment also reporting incidents of sexual assault. Shockingly, 50 per cent of respondents reported being leered at or subjected to suggestive gestures, while a quarter faced unwelcome sexual advances or demands for favors. The most common complaint, voiced by 61 per cent of those affected, was the experience of unwanted crude banter or jokes in the workplace.

The Pharmaceutical Services Negotiating Committee (PSNC) has called for improved workforce planning to be undertaken by the government and NHS. However, for this to happen, it added, "high quality workforce data for community pharmacy needs to be available." For this reason, the Department of Health and Social Care (DHSC), NHS England and PSNC have agreed that it will be a Terms of Service requirement for contractors to complete the annual Health Education England (HEE) community pharmacy workforce survey, which will then provide a full picture of the community pharmacy workforce, including identifying the number of vacancies and regions where these are particularly hard to fill. PSNC said: "It is acutely aware of the challenges that contractors and their teams are currently facing, so in recognition of the workload associated with completing the annual workforce survey, we have negotiated with DHSC and NHS England that the requirement to undertake an annual patient satisfaction survey will be removed from the Terms of Service from 1st October 2022." This means that contractual requirement will no longer apply to contractors in 2022/23 and going forward.

The Royal Pharmaceutical Society (RPS) has published its general election manifesto highlighting some of the key challenges the next Government will face to support patients and the NHS. The manifesto calls on general election candidates to show their support to securing patient access to medicines, a better-connected NHS, enhancing accessible prescribing in local communities, and maximising health improvement through locally accessible pharmacy teams. Candidates are urged to called on the next Government, if elected, to support the pharmacy workforce to deliver patient care, unlock the potential of new advances in medicines, and continue to support funding for the UK science and research community. The manifesto highlights the need for investment in IT infrastructure to support a better-connected NHS to enable all health professionals to access and update a patient's record.
