The Community Pharmacist Consultation Service (CPCS) will be expanded to enable urgent and emergency care settings to refer patients to a community pharmacist for a consultation for minor illness or urgent medicine supply from Monday (15 May), the DHSC and NHSE said. The service builds on the progress made in GP referrals via the CPCS and hospital referrals under the Discharge Medicine Service. It was originally planned to launch in March, and fee for this service will be the existing CPCS fee of £14, as per the agreement for both year 4 and year 5 of the Community Pharmacy Contractual Framework (CPCF) 2019 to 2024 5-year deal. In an update on the CPCF, published today (12 May), the Department of Health and Social Care (DHSC) and NHS England (NHSE) has also committed to the 4 October 2023 launch date for the Tier 2 of the Pharmacy Contraception Service, subject to a positive evaluation of the pilot. The Tier 1 of the service was launched on 24 April, delayed by over three months. This new service enables community pharmacists to provide ongoing management, via a patient group direction (PGD), of routine oral contraception that was initiated in general practice or by a sexual health clinic. The fees for this service are as follows: a fee for each consultation of £18; and a set-up fee of £900, paid in instalments. The Tier 2 will "enable community pharmacists to also initiate oral contraception, via a PGD, and provide ongoing clinical checks and annual reviews," Alette Addison, deputy director for pharmacy, dental and optical at the DHSC and Ali Sparke, director for dentistry, community pharmacy and optometry at the NHSE, said in a letter.

"This £200 million investment, assured by the Department of Health and Social Care as new and additional funding, should aid NHS leaders in their preparations and mitigation for what will be a seriously difficult winter period," said Matthew Taylor, Chief Executive of NHS Confederation. Dr Amanda Doyle has been appointed as NHS England as director of primary and community care. Prior to her new role, Amanda had joined NHS England and NHS Improvement as North West Regional Director on 2 August 2021 and previously she was the Chief Clinical Officer for West Lancashire Clinical Commissioning Group (CCG), Blackpool CCG and Fylde and Wyre CCG. The Pharmaceutical Services Negotiating Committee (PSNC) has launched a four point plan at its parliamentary drop-in event for MPs and peers hosted on Tuesday (15 November). The event lasted for two hours, during which MPs had the opportunity to drop-in and have their blood pressure measured by pharmacists with experience of working on the front-line and were briefed on the urgent need for action to address the pressures on the sector. Outline of the four-point plan: Resolve the funding squeeze: Community pharmacy needs an immediate funding uplift to prevent large-scale pharmacy closures, as well as emergency business relief to get through this winter. Tackle regulatory and other burdens: Pharmacies must be protected from medicines market shocks, supported to help them weather the workforce crisis, helped to free up capacity and freed from red tape that does not enhance patient care. Help pharmacies to expand their role in primary care: Pharmacies could do more to support the delivery of primary care for example through offering clinical services for long-term conditions - like hypertension, diabetes and respiratory disease, supporting people to adopt healthier lifestyles and prevent the development of long-term conditions, medicines optimisation, and a much wider variety of NHS vaccinations.

The Welsh chapter of the Royal Pharmaceutical Society, along with ten other royal colleges and professional bodies, have renewed calls for the urgent transformation of mental health services to address increasing demands on the NHS in Wales. Representatives of the Royal College Mental Health Expert Advisory Group joined members of the Senedd in Cardiff to outline the work of the group over the past year and to explain why a well-trained, fully supported and equipped multi-disciplinary workforce is critical for the development of care for people with mental health conditions. The recent 'Delivery plan for recovering access to primary care' announced a 'Pharmacy First' service for England, mirroring similar approaches in Scotland and Wales. The association has estimated that with the added capability to supply non-prescription medicines and prescribe additional prescription-only medicines, an ambitious 'Pharmacy First' service could free up 30m+ GP appointments annually. Harnessing community pharmacies to deliver care for minor health conditions will effectively create 11,000 urgent care centres in England.

The National Pharmacy Association (NPA) highlighted the role played by community pharmacies during pandemic to provide urgent care and vital support to people with long-term medical conditions at its opening remark in the Covid-19 public inquiry held on Tuesday (28 February). NPA is the core participant in the Covid-19 public inquiry. lawyer Brian Stanton made an opening statement on the NPA's behalf which focused on three areas- health inequalities and the needs of vulnerable patients; the impact of medicine shortages and medicine price increases and the challenge that community pharmacy faced in responding to the pandemic and maintaining patient services following long-term under investment. Stanton said: "The UK's community pharmacies were on the frontline of efforts to limit the impact of coronavirus and to keep people well, and as well as handling a massive increase in demand for healthcare advice and medicines, they also continued to provide urgent care and vital support to people with long-term medical conditions. "However, there are now very many at risk of closure during to underfunding and when the Inquiry comes to consider its recommendations the NPA would encourage you [the presiding judge] to think about how resilience can be built into future plans." The statement included a compelling account of the commitment typical of so many pharmacies during the pandemic - from husband and wife Pete and Sukhi Johal, both NPA members and pharmacists, who co-own Calow Pharmacy in Chesterfield.

The Parliamentarians have called the government to urgently consider the short-term funding to stabilise the community pharmacy sector. 39 MPs and Peers from across the political spectrum have signed letters to the Health Secretary, Steve Barclay MP and raised their growing concern about the pressures facing England's community pharmacies and the risks this poses to patient care and safety. "Steve Barclay MP received correspondence from 23 Opposition MPs, with six Peers also signing a letter. Separately, 9 Conservative MPs also wrote to the Health Secretary and up to 10 others have signalled their intention to write to the relevant Minister," said the Pharmaceutical Services Negotiating Committee (PSNC). The Parliamentarians acknowledged that pharmacies have the potential to do more to support the NHS and they called for the government to urgently consider a commitment to working with the sector to create a fair and fit for purpose future funding model that ensures sustainability and properly funds pharmacies for what they do. All signatories also called for the introduction of a fairly-funded Pharmacy First service.

The Royal Pharmaceutical Society (RPS) has been on the lookout for innovative ideas and suggestions that could transform the future of pharmacy practice in England over the next decade. The society's new project with the King's Fund to transform the future of pharmacy practice in England is at a consultation phase, seeking views and opinions of pharmacy teams from all areas of practice including primary, secondary, social and community care to ensure that the system gets the best out of pharmacy and the public receives seamless, joined-up care. "We want to build a vision that sets out the role of and value of pharmacists and pharmacy teams working across systems, providing patient care and NHS services," said RPS in a statement. "Transforming the future of pharmacy practice recognises the urgent need to build on new ways of working established across health and care systems during the pandemic to meet the increasingly complex health needs of people and improve patient outcomes."

A recently published report on medicines optimisation highlights the urgent need for transformative changes across all pharmacies to unlock their full potential in delivering integrated care. The report by Public Policy Projects (PPP) is first of the four in the series that emphasises the significant benefits that medicines optimisation can bring to integrated care systems (ICSs). This includes reducing health inequalities and improving population health. Pointing to the "untapped opportunity" that medicines optimisation presents for ICSs facing financial challenges, the report underscores the vital role of pharmacies in enhancing patient outcomes. The launch of the Pharmacy First services has already demonstrated the essential contribution of pharmacies to the health and care system. With "medicines being the second highest cost to the NHS," the report recommends ways "to achieve better value for money while improving patient outcomes".

Prime Minister Rishi Sunak was questioned whether he 'will take urgent action to prevent pharmacy closure'. At Prime Minister's Questions (PMQs) in Parliament held on Wednesday (26 April) he replied, "we will continue to do everything we can to support community pharmacies." Labour MP for Knowsley and All-Party Pharmacy Group member, George Howarth said: "Given the chronic lack of capacity in the NHS, the Prime Minister will be aware that community pharmacy can help deal with minor illnesses. But there is a problem on average 10 Pharmacy close every month in England." Prime Minister was further asked "will he take urgent action to prevent further closing and commission a properly funded 'Pharmacy First' service for minor illnesses? Sunak replied: "I've been a wholehearted champion and believer in the role that community pharmacists can play. we want to make sure that they can do everything to ease some of the pressures in primary care.

The NHS has made major improvements across long waits, urgent and emergency services, and cancer care, latest performance figures published on Thursday (11) have shown. The number of patients waiting more than 18 months fell to 10,737 by April - down by more than 90 per cent from 124,911 in September 2021 and by more than four-fifths since the start of January when there were 54,882. Around half of NHS trusts in England have no patients on their elective care waiting lists apart from those who have chosen to wait longer. More than one in five (21%) trusts had completely eliminated 18-month waits. However, the overall waiting list has risen to over 7.3 million entries in England. "I promised I would cut NHS waiting lists and we are delivering," Prime Minister Rishi Sunak said. "Reducing 18-month waits by over 90 per cent is huge progress, and it is testament to the hard work of NHS staff who have achieved this despite one of the busiest winters on record. "We still have work to do, but backed by record government investment and the ongoing efforts of the NHS, I am confident we will get patients the care they need more quickly." Ambulance response rates have improved to their fastest in two years, with average category two response times now at 28.5 minutes and category one at 8 mins. The 62-day cancer backlog has fallen for the first time since before the pandemic, with those waiting two months or more down from 21,823 at the end of the last financial year (March 2022) to 19,248 at the end of this March (2023). This is down almost 15,000 from a peak of 34,000 in July 2022.

The Company Chemist Association (CCA) highlighted its concerns around the recruitment of pharmacists into Primary Care Networks as well as the need for additional investment. Commenting on the Ipsos survey findings, the Malcolm Harrison, the Chief executive of the CCA said: "We welcome the findings of the Ipsos survey, confirming that patients value the quality of service and advice community pharmacies routinely provide. "We are very concerned however that this cancer detection pilot, and all other pharmacy services, are at risk if the NHS is not prepared to inject urgently needed funding into the sector." He stated that without substantial investment, "we will see the continued erosion of the service pharmacies can provide." In addition to the need for critical additional funding the association is also calling on the NHS to pause the recruitment of pharmacists into GP surgeries. He said: "Patients are suffering because the demand for pharmacists in England is now significantly greater than that which the existing workforce can deliver."

The National Pharmacy Association has published a medium-term prospectus for the development of pharmacy services, calling it a challenge to old ways of thinking and an opportunity to redefine the sector's role in the NHS. It follows months of dialogue with NPA members about what a clinical future could look like for the sector. The document - Making Changes Meeting Needs - will be shared with the King's Fund and Nuffield Trust who are currently working on a new vision for the future on behalf of Community Pharmacy England. NPA chair, Nick Kaye, said: "Building out from the existing portfolio of services, there are some major opportunities within this decade, encompassing prevention, medicines optimisation, long term medical conditions and urgent care.
