Community Pharmacy England (CPE) is gearing up to conduct a vital Pharmacy Advice Audit, calling on pharmacy owners and their teams to help measure the
increasing reliance on pharmacies within communities.
With years of financial strains impacting the sector, despite the much-needed £645 million recovery plan funding, the sector finds itself on the edge.
The launch of Pharmacy First sparked a notable surge in people seeking advice from community pharmacies, both within and beyond the funded service, as revealed
by recent sector polling.
To accurately assess this trend and advocate for sustainable funding, CPE is initiating an audit to gather data crucial for showcasing the extent of unfunded
care provided by community pharmacies.
Scheduled to commence this week, the Pharmacy Advice Audit will entail pharmacy teams documenting informal healthcare advice dispensed over a single day.
Prime Minister Rishi Sunak, during today's (15th May ) Prime Minister's Questions (PMQs), reiterated his commitment to supporting community pharmacies amidst
growing concerns over pharmacy closures.
Addressing questions regarding the Pharmacy First initiative, Sunak assured the House of Commons of his dedication to bolstering resources for these vital healthcare
providers.
In response to a query raised by Member of Parliament, St Ives MP Derek Thomas, citing statistics from the National Pharmacy Association (NPA) about "committing to
directing funds towards Community Pharmacy to alleviate financial pressure and prevent closures, Sunak remarked that he "cares deeply about the future of community
pharmacies."
Sunak acknowledged the indispensable role played by community pharmacists in alleviating pressure on urgent care services, emphasising the importance of initiatives
like Pharmacy First in government's commitment to supporting community pharmacies.
"There are over 10 and a half Community pharmacies across the country working incredibly hard to serve their patients," Sunak said praising the community pharmacists
and highlighted the concerning trend of pharmacy closures, citing a significant increase compared to the previous year.
To maximise the benefits of the Pharmacy First service for patients, the NHS, and community pharmacies, Community Pharmacy England (CPE) has advised the
community pharmacists to to establish a robust relationship between community pharmacies and local general practices.
The news comes after several reports highlighted issues regarding GP referrals for the seven minor ailment conditions under the Pharmacy First services.
According to the reports, many pharmacists claimed that "many GPs in England are not sending patients to them to be treated - and that some are refusing to
participate at all in the "groundbreaking" Pharmacy First scheme".
In response to foster better relationship with the GPs, CPE has said that strong local collaborations will enable general practice teams to confidently refer
patients to the Pharmacy First and other community pharmacy services where appropriate.
Moreover, the organisation emphasised implementation of long-term changes in practice will take time. The current focus on integrating GP referrals to Pharmacy
First will likely require sustained effort over several months or longer.
A group of 21 community pharmacies in the Midlands and Yorkshire will have a new name - Pickfords Pharmacy.
The group has been created by combining three independent pharmacy businesses which traded as Mr Pickford's, McGills Pharmacy and D&R Sharp.
All pharmacies in the new group will be rebranded over the coming months, with a series of launch events planned by local teams.
Speaking after the unveiling of the first branch to receive new signage in Hexthorpe Doncaster, Mimi Lau, Pickfords Pharmacy's chief operating officer, said:
"This is a turning point for the group, with all our pharmacies operating under one name with a consistent, contemporary brand.
"Initial reaction has been fantastic and very soon the name Pickfords Pharmacy will be widely recognised for the great service we deliver to customers and patients
every day."
Speaking of the consolidation, Nick Yarrow, the newly appointed group chief executive officer, commented: "Since the enlarged group came together last year, we have
been striving to ensure that the best elements of each business have been retained for the benefit of our customers and team members.
"This is part of our journey to be more than just a pharmacy - we want to be a trusted source of healthcare services at the heart of each one of the communities we
serve".
The Pharmaceutical Services Negotiating Committee (PSNC)'s 2022 Pharmacy Advice Audit revealed that more than 1.2 million consultations a week - or 65 million
a year - are now being carried out by community pharmacy teams in England.
This is an increase from 2021, when the audit results suggested that in total pharmacies were providing 58 million consultations per year.
PSNC has published the findings of the audit of over 4,000 community pharmacies carried out earlier this year. During the audit, 82,872 informal patient consultations
were recorded, with the average pharmacy completing 19 consultations per day.
This suggests that more than 1.2 million informal consultations are taking place in community pharmacies in England every week.
The audit helped to quantify the number of informal referrals being made to pharmacies by GPs and NHS 111, with 7,774 informal patient referrals into pharmacy
coming from these routes; grossed up to a national level that means 117,000 cases per week.
These are all referrals that could and should have been made by the NHS Community Pharmacist Consultation Service (CPCS).
Ensuring comprehensive healthcare services for the community necessitates collaboration between pharmacies and GP surgeries. However, several pharmacists
have voiced concerns about a lack of cooperation from their local GPs in implementing the Pharmacy First (PF) service.
In a survey conducted by the National Pharmacy Association (NPA) covering 470 pharmacies across England, three-quarters of pharmacists (77 per cent) indicated
that local GP practices are not appropriately referring patients to the new service.
The survey highlighted instances of patients either not being referred or being referred but found ineligible for support through Pharmacy First. Additionally,
one in five NPA members (19 per cent) felt that their local GP practices were not engaging with the initiative at all.
Commenting on the issue, NPA chair Nick Kaye said: "Feedback from across our network suggests that the rollout of Pharmacy First varies considerably from area
to area.
YouGov polling commissioned by the Pharmaceutical Services Negotiating Committee (PSNC) has revealed that majority of the people responding to the poll said
that they would be concern by the pharmacy closure in their locality.
The results show the extent of the public backing for a Pharmacy First approach - PSNC has been arguing that a fully-funded service would be beneficial for pharmacies,
patients and the wider NHS.
The poll witnessed 1,774 adults' participation from England. It showed 68 per cent of people agree that they would find it easier to seek health advice for common
conditions, such as skin conditions or indigestion, at a community pharmacy rather than a GP surgery. Only 11 per cent disagreed that this would be easier.
While 75 per cent of people agree that they would like to see community pharmacies offering more healthcare services such as treating urinary tract infections or
sore throats. Only 6 per cent strongly disagreed with this.
When asked how important, if at all, are the services provided by community pharmacies to them, their family, or community, 78 per cent indicted this was very
important. Only 14 per cent said this wasn't important.
77 per cent said they would be very concerned if the pharmacy they use most often were to close permanently. Only 19 per cent said they would not be very concerned.
As part of a new blueprint for primary care, the government today (May 9) announced an investment of £645 million over two years to expand community pharmacy
services in England.
In a statement, NHS England said: "For the first time ever, patients who need prescription medication will be able to get it directly from a pharmacy, without a
GP appointment, for seven common conditions including earache, sore throat, or urinary tract infections."
Prime minister Rishi Sunak hopes that the measures will help end the "all-too stressful wait on the end of the phone for patients" by freeing up 15 million slots at
doctors' surgeries over the next two years.
"We will end the 8am rush and expand the services offered by pharmacies, meaning patients can get their medication quickly and easily," he said
Almost half a million women will no longer need to speak to a practice nurse or GP to access oral contraception and will instead be able to pop into their local
pharmacy for it, according to the government announcement.
Blood checks for people suffering from moderate risk of heart attack or stroke conducted in community pharmacies will more than double from 900,000 last year to
2.5 million next year.
NHS chief executive Amanda Pritchard said the "ambitious package" would help transform how care is provided within the health service "with pharmacies playing a
central role in managing the nation's health including providing lifesaving checks and medication for common conditions for the first time.
The nationwide implementation of the Pharmacy First service is viewed as a significant development that benefits pharmacies, primary care, and the broader
National Health Service (NHS). Pharmacy teams are striving to ensure its success despite challenges within the industry landscape. However, not all pharmacists
are able to reap the benefits from it.
Recently, two members of Numark shared their experiences of how the scheme is impacting their pharmacies.
Graham Phillips, who owns Letchworth Pharmacy in Hertfordshire, revealed that patients are responding favourably to the service. "It's all good, people are just
delighted to have easier access to care," he said.
In its first month, Letchworth Pharmacy provided around 100 consultations, and Phillips envisions that as the service matures, they will be able to conduct an
average of 200 interventions per week.
"That will make it financially viable for us and mean that we're acting at scale from an NHS perspective. That's 200 GP appointments we've freed up in a
week - that's a huge capability and capacity bump for the NHS," he added.
Graham credits part of his success with Pharmacy First to the strong relationship he has established with local GPs in his area.
"If we stand still, we will die. We have to keep moving, and we have to keep changing" said Patrick Gompels, co-owner of Gompels Limited, while emphasising
the need for innovation in community pharmacy services, during the recent Pharmacy Business Conference 2024.
Centered on the theme 'Pharmacy of Tomorrow,' the conference, held at the Hilton Wembley on April 28, focused on adopting and adapting innovative technology as
well as new ways of working and thinking to enhance patient services.
Patrick was joined by Mayank Patel of Pearl Chemist Group and Michael Lennox, CEO of Community Pharmacy Somerset (LPC), during the panel discussion on innovation
in community pharmacy services, moderated by Reena Barai, pharmacist and owner of S G Barai Pharmacy.
On the changes he made at Gompels, Patrick said: "The first thing we did was a complete overhaul of all of our systems. Pretty much everything was stripped down."
He shared that Gompels has embraced innovation by partnering with forward-thinking companies like Titan PMR, Drug Comparison, and Real World Analytics to enhance
their processes and data analysis capabilities.
Pharmacy Minister, Will Quince received flu vaccination and blood pressure check at his visit to a Boots pharmacy in Westminster arranged by the Company
Chemists' Association (CCA).
The Boots pharmacist, Flavia, explained to the Minister the broad range of services the pharmacy delivers, from the new medicines service to emergency contraception
and the community pharmacy consultation service.
The Minister witnessed first-hand much of what the community pharmacy sector has to offer patients and the NHS.
Malcolm Harrison Chief Executive of the CCA said: "We share the Minister's vision on how community pharmacies and their teams can contribute to the 'Plan for
Patients' set out by the Secretary of State. It is great to have the support of the Minister and we are excited for the future of pharmacy.
A recent analysis conducted by the Company Chemists' Association (CCA) has revealed stark disparities in patient engagement with the Pharmacy First
service across the country, likening the pattern to a 'patchwork quilt'.
It indicated that patients in the highest-performing Integrated Care System (ICS) received seven times more Pharmacy First consultations than those in the
lowest-performing regions.
As of April 21, more than 3,000 CCA member pharmacies had delivered 126,484 Pharmacy First consultations since the programme's launch.
However, the analysis found significant regional variations, with nearly 13,000 consultations completed in the North East and North Cumbria ICS, while only
948 were completed in Gloucestershire ICS.
The CCA calculated that if every ICS had as many consultations as the highest-performing ICS, there would have been an additional 600,000 Pharmacy First
consultations nationwide.
With the UK General Election set for 4 July, pharmacy bodies are actively engaging with all political parties to secure support for the community pharmacy
sector. They are pressing parties to address urgent issues such as pharmacy closures and medicine shortages.
"The upcoming general election is a critical opportunity for us to strengthen supporters of community pharmacy in readiness for the new Parliament," said Janet
Morrison, chief executive of Community Pharmacy England (CPE).
She stressed the need for unity within the sector in advocating for community pharmacy and raising awareness of the issues with election candidates from all parties.
As the general election approaches, CPE has called upon all political parties to support pharmacies by "introducing a long-term sustainable funding model and
reviewing the medicines supply chain, including much-needed short-term relief measures."
The health secretary, Steve Barclay was asked over the delay of 'Pharmacy First' model in England that was proposed by the Pharmaceutical Services Negotiating
Committee (PSNC) last year, at the Health Committee evidence session held on Tuesday (31 January).
Taiwo Owatemi MP (Chair of the Pharmacy APPG) asked whether he was adhering to his statutory responsibility to ensure continued access to medicines and cited the
CCA's closures in areas of deprivation research.
Barclay replied that the government is 'investing more'. He said, "We put an extra 100 million on top of the 2.6 billion a year we commit to community pharmacy to
expand the range of clinical services. We've got over 2 million patients that have been referred to community pharmacy from NHS."
He further added: "One of the issues I'm very keen on is to explore what more we can do in pharmacy not least given the pressures on GP and the opportunity to look
at what it is currently people go to GPS for where potentially the risk to do more at the pharmacy and we're already doing that."
Community Pharmacy England (CPE) has taken an interesting step to increase public awareness and understanding of the Pharmacy First service.
The organisation has launched a new video animation that provides clear and concise information about the service, highlighting the seven conditions covered,
the eligible age ranges within the clinical pathways consultations, and explaining how patients can access these services.
Pharmacy owners and their teams are encouraged to use the new animation with existing resources to communicate the benefits and scope of the service to their
patients.
The animation can be linked to pharmacy social media content and downloaded for display on digital screens in pharmacies. General practices are also encouraged to
use the animation on digital screens in their practice waiting areas.
Digital pharmacy app Charac has raised over £1 million in debt and equity, bringing total funding to date for the London-based startup to £2.5m.
Pharmacy Business understands while the majority of the new finance, worth about £1.2 million, comes from the National Pharmacy Association (NPA) coffer, the
remainder is put forward by a number of individual pharmacy businesses.
The pharmacy sector is in a state of crisis, experiencing nearly two closures per week over the past two years, Charac said in a statement.
According to data from the NHS Business Services Authority, there are now only 11,026 community pharmacies in England, the lowest number since 2015. The current
crisis is attributed to escalating operational costs, a shortage of staff, and diminished government financial support.
"Discussions must involve the government on behalf of NHS Scotland, CPS on behalf of the owners, and the PDA as the pharmacists' representative," it said.
The association believes that there is a need for discussion and decision-making that listens to and balances the rights and responsibilities of both employers
and workers, to generate benefits for individuals, organisations, and society.
It added: "Even though Scotland provides the most generous community pharmacy settlement in the UK, recent reports suggest it is not enough for pharmacy owners
with CPS's rejection of the latest funding proposal in May. Though the Scottish government found an extra £20M to ease pressures related to medicines price
increases, an overall agreement has still not been reached."
"The UK-wide chains may be doing less well in the parts of their networks covered by the Westminster government's contract, but the taxpayers and government of
Scotland need to be given reassurance that they are in no way subsidising funding shortfalls in England's pharmacy contract."
Recently, when LloydsPharmacy's Scotland branches recently came on the market, they appear to have been sold exclusively to existing contractors, including the
UK-wide multiple, Rowlands Pharmacy, who have acquired 30 of them. Other small and medium-sized Scottish pharmacy chains have apparently been able to double in
size overnight by acquiring branches.
Prime Minister Rishi Sunak was questioned whether he 'will take urgent action to prevent pharmacy closure'.
At Prime Minister's Questions (PMQs) in Parliament held on Wednesday (26 April) he replied, "we will continue to do everything we can to support community
pharmacies."
Labour MP for Knowsley and All-Party Pharmacy Group member, George Howarth said: "Given the chronic lack of capacity in the NHS, the Prime Minister will be aware
that community pharmacy can help deal with minor illnesses. But there is a problem on average 10 Pharmacy close every month in England."
Prime Minister was further asked "will he take urgent action to prevent further closing and commission a properly funded 'Pharmacy First' service for minor
illnesses?
Sunak replied: "I've been a wholehearted champion and believer in the role that community pharmacists can play. we want to make sure that they can do everything
to ease some of the pressures in primary care.
Will Quince, MP for Colchester and Health Minister visited Rowlands pharmacy on Friday (20 January) in Nayland Road in his constituency.
The important role played by pharmacies in providing NHS care, support, and advice helping the local community to live healthier lives and manage long-term conditions
were discussed among the pharmacy teams.
Mr Quince said: "Community pharmacy is playing an increasingly important role in ensuring people in Colchester have convenient walk-in access to the healthcare
support and advice they need.
Pharmacies are not simply places to collect prescriptions, they provide a wide range of services and are the third pillar of access to primary care alongside GPs and
A&E. I would urge people to think pharmacy first if they have a minor ailment; need advice on taking their medication; want to stop smoking or want to check their
blood pressure."
Lo's Pharmacy, a Yorkshire-based pharmacy chain is adopting a new approach to hub and spoke in a first of a kind model to be used in the UK.
The pharmacy will centralise its repeat prescription dispensing service for original packs with the support of its medication wholesaler.
The independent group, which has 25 NHS community pharmacies across Yorkshire, will install Centred Solution's Automated FLOWRx Hub product at its central dispensing
facility in Wath-Upon-Dearne, South Yorkshire.
But unlike other FLOWRx customers, who pick packs from a robotic dispensing system or internal warehouse, Lo's Pharmacy will receive patient-specific flow totes
directly from their wholesaler which can then be processed by their hub.
This workflow has not been used anywhere else in the country until now and it is a revolutionary approach to hub and spoke dispensing, showing what can be achieved
when key players decide to collaborate. It further demonstrates that there are range of variations of hub and spoke models available and accessible to independent
and multiple community pharmacies across the UK.
