DENliest Catch: STAR Recruitment Form
Please complete this form if you have recruited a new STAR Discovery Educator into the DEN community during DENliest Catch (September 26 - December 1, 2008). At the end of the season we will check to make sure that your new recruit has completed all parts of the application process (online portion and event report) and is officially a STAR DE. At that point, we will send out DVDs. Remember, simply completing this form does not guarantee that you will receive your DVDs. Your recruit must officially be a STAR DE by December 1, 2008.
