This toolkit can help planners, public health professionals, and the general public include health in the community planning process. Developed in partnership between the American Planning Association's Planning and Community Health Research Center and the Centers for Disease Control and Prevention's Healthy Community Design Initiative, the toolkit is composed of four elements that work together to achieve this goal:

Everyone wants good health in order to be productive at work and to succeed in school, and to have affordable medical and housing costs. To make this opportunity a choice for all Minnesotans, we all have to do our part in creating and maintaining healthy homes and communities. Every person and every organization has a role to play, small or large.

Community Health Centers serve the primary health care needs of more than 22 million patients in over 9,000 locations across America. They play a crucial role in the nation's health care system, providing affordable health services for millions of uninsured, the working poor and newly jobless Americans. Health centers are good for the country. They lower overall health care costs, improve the health of their patients and generate economic opportunities in the communities they serve by providing jobs and training for local people. Community Health Centers create savings in health care every time a patient opts for an exam and treatment at the first sign of a health issue instead of waiting until a costly emergency room visit or hospitalization is the only option. Each health center takes a tailored approach to meet the unique needs of the people in its surrounding community. That local approach to health care, combined with an emphasis on comprehensive preventative care, generates $24 billion in annual savings to the health care system - for the American taxpayer, local, state and federal governments and public and private payers alike.

The Strong Healthy Communities Initiative (SHCI) in Newark, NJ, is a cross-sector institutional partnership that aims to close the education "opportunity gap" in Newark by improving child health and well‐being as a lever to greater student achievement. It is the vision of SHCI and our partners to ensure that all Newark children, regardless of income, are healthy and ready to learn.

Industrial food production, which currently dominates U.S. agriculture, is a dead end. It damages air, water and soil, harms rural communities, and limits future productivity. But there's a better way. Scientists call it agro-ecological agriculture. We call it healthy farms. Healthy farms can be just as productive as industrial farms-and a lot more sustainable.

Lifting Up What Works is our way of focusing attention on how people are working successfully to use local, state, and federal policy to create conditions that benefit everyone, especially people in low-income communities and communities of color. We share our findings and analysis through our publications, website and online tools, convenings, national summits, and in briefings with national and local policymakers. Our work is grounded in the conviction that equity - just and fair inclusion - must drive all policy decisions.
