Two Great Tools to Create Image Based Quizzes ~ Educational Technology and Mobile Learning.  I like the one that uses Google Docs.  You can  probably include graphics for answers to make the quiz more UDL

Google Keep  is an Android and Google App.  It is a to do list manager but it now has added OCR capabilities

You can filter the Add Ins and just look at those for Education.  There is a free Add in from Text Help that will help you collect your highlights of the document. Teachers can create documents and add the Add ins the students need.
